The Health Guide helps Navajo County Health Plan members spend less on high quality healthcare!
Your Health Guide can assist with obtaining care at reduced or zero cost!
Here's how it works:
Step 1: Talk to your doctor or primary care provider
If you need to have an image or procedure done, the Health Guide can help you!
Step 2: Call the Health Guide- (928) 524-4039
OR email: HealthGuide@navajocountyaz.gov
-You must call BEFORE you schedule the procedure.
Step 3: The Health Guide will work with you to find the best option for you.
And the best part is:
We share the savings with you!
-Members can earn up to $250 for imaging, or up to $3,500 for a procedure!
Note: This program is not eligible for urgent or emergency care services. The time to schedule images typically ranges from 2-5 business days, while scheduling consults for surgeries or procedures can take from 15-30 business days.
